Privacy

Privacy Policy.

How NookMeds handles personal information and how clinical privacy information is managed through the OpenLoop provider network.

1. Who we are

What this policy covers

This Privacy Policy explains how PRIMEWELL INTEGRATE HEALTH LLC, doing business as NookMeds (“NookMeds,” “we,” “us,” or “our”) collects, uses, discloses, retains, and protects personal information when you visit nookmeds.com, interact with our communications or customer support, use our non-clinical technology and administrative services, or otherwise interact with us (collectively, the “Services”).

NookMeds provides a non-clinical technology and administrative platform. Clinical services are provided by independent licensed clinicians and the applicable professional entity or entities participating in the OpenLoop provider network. Clinical decisions, including whether treatment is appropriate, are made solely by the treating clinician.

2. Clinical privacy

OpenLoop clinical records and PHI

If you receive clinical services through a Provider in the OpenLoop provider network, your medical records and protected health information (“PHI”) are subject to the applicable Provider’s privacy practices and legal obligations. The OpenLoop Notice of Privacy Practices describes the privacy practices of the OpenLoop Affiliated Covered Entity, patient rights, and how to submit a privacy complaint or request.

NookMeds stores and transmits health-related information in connection with its Services. Where NookMeds handles PHI on behalf of an applicable covered entity, it will do so only as permitted by the applicable business associate agreement, service agreement, authorization or consent, and law.

NookMeds is not stating that it is a member of the OpenLoop Affiliated Covered Entity. Nothing in this Policy changes the privacy notice, medical-record rights, or clinical obligations of an applicable Provider.

3. Information we collect

Information provided, observed, and received

Information you provide. We may collect information you provide directly to us, such as your name, email address, telephone number, postal address, date of birth, account credentials, communications, support requests, and preferences. If you use an intake, request care coordination, communicate with us about care, or otherwise submit health-related information, we may collect information about your health history, symptoms, medications, treatment goals, insurance or payment information, and other information that may be health information or PHI, depending on the circumstances.

Information collected automatically. When you visit our website or use our Services, we may collect device and usage information such as IP address, device and browser type, operating system, referring URLs, pages or features viewed, dates and times of access, interactions with our pages, and diagnostic or security logs.

Information from service partners. We may receive information from Providers, pharmacies, payment processors, identity-verification providers, customer-support vendors, communications providers, analytics providers, and other service providers when they help us operate the Services.

4. How we use and disclose information

Supporting care, operations, and security

We may use information to operate, maintain, secure, and improve the Services; create and administer accounts; respond to inquiries and provide support; communicate about intake status, appointments, service changes, billing, pharmacy coordination, or operational matters; facilitate care coordination with Providers and pharmacies; process payments; prevent fraud; comply with legal and regulatory obligations; and protect the privacy, safety, and security of NookMeds, users, Providers, pharmacies, and others.

We may disclose information as permitted by law and as necessary to provide the Services, including to Providers and professional entities in the OpenLoop provider network, pharmacies, service providers, governmental or legal authorities when required, parties involved in a business transfer, and others at your direction or with your consent.

We will not use health information for marketing, sale, or other purposes requiring authorization unless we have the required authorization or another lawful basis to do so.

5. Analytics and advertising technologies

Cookies, analytics, and pixels

NookMeds uses cookies, web beacons, local storage, analytics tools, the Meta Pixel, and other advertising pixels or similar technologies. These technologies may collect online identifiers, device and browser information, IP address, referral source, pages viewed, interaction events, and other usage information. We use them to operate and secure our website, understand usage, measure the performance of our advertising, and improve our Services.

Depending on applicable law, these technologies may constitute a sale, sharing, targeted-advertising, or processing activity that gives you a right to opt out. NookMeds will provide the notices, consent choices, opt-out mechanisms, and limitation rights required by applicable law after its final tracking and state-law review is complete.

6. Your rights and choices

Privacy requests

Depending on your location and the information involved, you may have rights to request access to, correction of, deletion of, or a portable copy of certain personal information; to opt out of certain processing such as sale, targeted advertising, or profiling; to limit certain uses of sensitive personal information; or to appeal a decision regarding a privacy request.

For PHI held by an applicable Provider, the rights and request process are described in the applicable Provider’s Notice of Privacy Practices. For information controlled by NookMeds, submit a request by emailing privacy@nookmeds.com or writing to Privacy Office, PRIMEWELL INTEGRATE HEALTH LLC, 12636 High Bluff Drive, Suite 400, San Diego, CA 92130, United States. We may verify your identity before acting on a request. To appeal a privacy-rights decision, email privacy@nookmeds.com with the subject line “Privacy Rights Appeal.”

7. Security, retention, and children

Safeguards and limits

NookMeds uses administrative, technical, and physical safeguards designed to protect personal information against unauthorized access, use, disclosure, alteration, and destruction. We retain information for as long as reasonably necessary to provide the Services, comply with legal and contractual obligations, resolve disputes, enforce agreements, and maintain appropriate records. Retention periods may vary based on the information, purpose, legal requirements, and applicable medical-record obligations.

The Services are not directed to children under 18 unless NookMeds expressly states otherwise. If you believe a child has provided personal information to NookMeds without appropriate authorization, contact privacy@nookmeds.com.

8. Updates and contact

Keeping this policy current

We may update this Privacy Policy from time to time. We will post the updated version on this page and revise the “Last updated” date. When required by law, we will provide additional notice or obtain consent before a material change takes effect.
